PLEASE NOTE: We are NOT in network with AFFORDABLE CARE ACT (ACA)/EXCHANGE insurance plans. Please contact us if you have questions regarding the type of insurance our office accepts.
As a courtesy, we will bill your visit directly to your insurance company if you supply an updated proof of insurance at each visit. Please note that the insurance policy is a contract between you and your insurance provider. We are happy to file the claim for you as a courtesy, but any balance due is your responsibility. Recent changes in insurance coverage have resulted in the offering of high deductible plans and or higher copayments for services rendered. Please note ALL deductibles and copayments are due at the time of service and/or visit - no exceptions.
We accept Medicare patients at our practice and are a participating provider with Medicare. We do accept Medicare Assignment.
"Meaning of Assignment: acccepting assignment of the Medicare Part B payment means requesting direct Part B payment from Medicare program. Under an assignment, the approved charge, determined by the Medicare carrier, shall be the full charge for teh service covered under Part B. the participant shall not collect from the beneficiary or other perons or organization for covered service more than teh applicalble deductible and coinsurance."
As a courtesy, we will bill Medicare and also bill your secondary insurance provider directly. You will be asked to sign an Advanced Beneficiary Notice for services not covered under Medicare. You shall be accountable for your percentage of these services. Please be aware- Medicare does have an annual deductible and pays for 80% of all services. OUT-OF-POCKET AND UNINSURED PATIENTS:
The cash charges depend on the level of care that is needed and the complexity of the issues that have to be dealt with. Please note payment is expected at the time of service. We do not offer delayed billing for cash patients.
