In order to register and treat you promptly, our patients are required to bring current insurance information to each office visit. Please advise the front office of any changes in your insurance and/or contact information at your visit.
ALL COPAYMENTS, COINSURANCES AND UNMET DEDUCTIBLES ARE DUE AT THE TIME OF SERVICE. Due to recent strict regulatory laws, we can not bill for copayments. We accept cash, checks, money orders, and all major credit cards.
PLEASE NOTE: We are NOT in network providers for AFFORDABLE CARE ACT/EXCHANGE plans or limited (narrow) network plans. Information on the type of plans we do accept is listed under the "INSURANCE INFORMATION" tab on the left. Please contact us if you have questions related to the type of insurance our office accepts.
